A failed crossing in May was one of the most lethal engagements for Russia. But trapping the enemy is what the war in the east has largely boiled down to.

To make such crossings the army needs to have control of the space around it before they attempt it.  The Russians obviously failed to do that and I suspect it was because the command authority pushed them into attempting the crossing before properly preparing the battlespace for such a move.
