Many of the Russian commanders were killed

 NY Times:

Russia’s War Lacks a Battlefield Commander, U.S. Officials Say

Poor logistics and flagging morale have plagued a disjointed military campaign in Ukraine, according to American officials and independent analysts.

The Russian army has poor command and control from top to bottom.  It lacks NCOs and officers with a sound plan of attack.  It has poor communications that expose commanders trying to lead the troops.

It started with a plan that assumed little resistance.  When the resistance was there they seem to have no plan B.

The attack was further compromised by the shoddy equipment they gave the troops.  Recent complaints are of World War II-era rifles that are not reliable.  As many as 60 % of their cruise missiles were duds.  Their tanks and other mechanized units were vulnerable to advanced weapons provided by the US and UK.

Poor logistics also feed poor morale. The Russians had an exposed supply line with inadequate fuel or food.
