Who knew?

Washington Post:

In Libya, security was lax before attack that killed U.S. ambassador, officials say

I normally just use the "Who Knew?"  headline for the obvious, which does fit this one, but there is a larger question of who in the State Department was responsible for making sure this facility was defensible.
