Faulty gear made in prison supplied to troops

Washington Post:
Prison factory made faulty helmets for troops and no one was prosecuted

More than 149,000 U.S. military helmets made by Federal Prison Industries were recalled or quarantined, according to investigators who detailed the use of degraded armor and preselection of helmets for inspection to ensure approval.
It looks like a mistake to trust this work to a prison factory.  It appears that some in management may have put the troops at risk by colluding with those who made shoddy helmets.
